Vital Statistics

The Ashtabula County Health Department issues certified copies of birth and death certificates for persons who were born or who died in the Ashtabula County General Health District from 1909 to present date.  Certified copies of birth and death certificates have a raised seal and are signed by the Registrar of Vital Statistics.  The fee for a certified copy of a birth or death certificate is $19.00.  Certified copies can be obtained in person  Monday through Friday 8:00 a.m. to 4:30 p.m.   Uncertified copies are no longer available.

Certified copies may be requested by mail.  The written request must contain the full name at time of birth, date of birth, place of birth, and parents name.  Send a check or money order in the amount of $19.00 per certified copy,  payable to the Ashtabula County Health Department.  This request must also contain an original signature and a return address.   Submit written requests to:

Birth and death records are on file by place of occurrence.  The Ohio Department of Health houses original birth records for the State of Ohio from 1909 to present and original death records from1946 to present.   You may send your request to:

Birth and death records from 1867 to 1908 are filed at the Ashtabula County Probate Court, if recorded.

Births and deaths that occurred in Ashtabula City or Conneaut City are not kept at the Ashtabula County Health Department.
