Food Service/Food Establishments and Mobiles

Below is a link which will alow you to print out the application and the plan review information necessary to open a food service/food establishment or a mobile operation in the Ashtabula County Health District.

Also, below is a link for a transfer letter which is necessary if ownership of a food service operation is being changed.   Along with this completed transfer letter you will need to bring in the original license to the Health Department and a new license will be issued to you.

Temporary Food Service Operations

Below is a link which will alow you to print out the application and the plan review information necessary to apply for a Temporary Food Service License.   This license is only good for a 5 day consecutive event in the Ashtabula County Health District.
