Child and Family Health Service of Ashtabula County

The Child and Family Health Services (CHFS) program is designed as an organized community effort that develops programs to improve the health status of women and children in Ohio.

The goal of the CFHS Grant Program is to eliminate health deparities, improve birth outcomes, and improve the health status of women, infants and children in Ohio.

It is a basic function of public health agencies to assess health status, protect the health of the public and prevent disease.  In addition to the physical examinations provided, social service assessment and health education are provided for each client.

CFHS services are provided to eligibe persons in Ashtabula County regardless of income.  Payments are based on a sliding fee scale according to your income.  Healthy Start/Medicaid cards are accepted.  No one is denied service because of inability to pay.


CFHS Funded Agencies are made possible through a grant from the Ohio Department of Health in the interest of healthy children in cooperation with the Lead Agency, the Ashtabula County Health Department with services subcontracted with the Ashtabula City Health Department, the Conneaut City Health Department and the Family Planning Association of Northeast Ohio, Inc.

Well Child Exams Include:
 
  • Health history
  • Physical assessment
  • Nutrition assessment
  • Physical growth and development evaluation
  • Vision, hearing and speech screening
  • Blood tests for Anemia and Lead Poisoning
  • Urine and TB testing
  • Health and safety education
  • Assistances with referrals for identified needs/problems
